MIAMI, FL – Eighteen residents of Miami-Dade County are facing federal charges today following a sprawling investigation into a brazen health care fraud scheme that allegedly siphoned millions from private insurance companies. The operation, rooted in a network of shell companies, exploited vulnerabilities in Administrative Services Only (ASO) insurance plans offered by major employers.

Federal prosecutors allege that Reynaldo Castillo, 46, of Hialeah, spearheaded the conspiracy, alongside accomplices Hendris Castillo Morales, 33, of Miami, Lisbet Castillo, 23, of Hialeah, and Maite Garcia, 40, of Hialeah. The quartet allegedly controlled a staggering 30 companies across Miami-Dade County, used to submit fraudulent claims. They are accused of stealing the identities and licensing information of legitimate physicians to create the illusion of valid medical services rendered. The scheme targeted insurers including Cigna, Blue Cross Blue Shield (BCBS), United Health Care (UHC), and plans offered by companies like Miami-Dade County Public Schools, Pepsi Co., and others.

The indictment details how Alejandro Biart, 40, of Miami, allegedly accepted kickbacks for referring beneficiaries to the fraudulent clinics. These beneficiaries, according to the government, signed paperwork falsely claiming they received medical care they never actually received. Further compounding the alleged fraud, Maite Garcia is specifically charged with bribing beneficiaries to cover their monthly Cigna premiums, effectively silencing potential whistleblowers. Osvaldo Marin Medina, 48, of Hialeah, Humberto Martinez Rodriguez, 43, of Hialeah, Alejandro Jesus Cura, 47, of Miami, Dania Chavez, 43, of Miami, Ezequiel Severo Casas, 28, of Hialeah, Jose Gerardo Gonzalez, 23, of Miami, Julio Suarez, 47, of Miami, Nelson Ramos, 56, of Miami, Reinaldo Cinta Gonzalez, 46, of Miami, Rudy N. Dominguez, 25, of Hialeah, and Diulys Martinez, 39, of Miami, are all charged with agreeing to have shell companies established in their names, opening bank accounts, and processing fraudulent checks from the insurance companies – all for a fee.

All eighteen individuals – Reynaldo Castillo, Hendris Castillo Morales, Lisbet Castillo, Maite Garcia, Osvaldo Marin Medina, Alejandro Biart, Alejandro Jesus Cura, Dania Chavez, Ezequiel Severo Casas, Humberto Martinez Rodriguez, Jose Gerardo Gonzalez, Julio Suarez, Nelson Ramos, Reinaldo Cinta Gonzalez, Rudy N. Dominguez, and Diulys Martinez – face charges of Conspiracy to commit Health Care Fraud and Health Care Fraud (Case No. 15-20144-Cr-Scola). Maite Garcia additionally faces charges of Conspiracy to Pay Health Care Kickbacks and Payment of Kickbacks in Connection with a Federal Health Care Program. The U.S. Attorney’s Office for the Southern District of Florida, led by Wifredo A. Ferrer, has vowed to pursue the maximum penalties for those convicted.
