Chicago’s grim underbelly of healthcare fraud just got a new inmate. Gregory D. Toran, 68, of Hazel Crest, Ill., was sentenced to five years in federal prison for orchestrating a $4.7 million scam against Illinois’ Medicaid program. The feds say Toran, co-owner of IBT Transportation, LLC, systematically overbilled for non-emergency medical transport services that were either never provided, grossly exaggerated, or billed for dead patients.
Toran didn’t act alone. He ran the scheme with Tina Kimbrough, 44, of Berwyn, Ill., who was sentenced in August to 30 months in prison after pleading guilty to conspiracy. Kimbrough was also ordered to pay $4 million in restitution—jointly and severally with Toran. The duo exploited Medicaid’s billing system from December 2005 to June 2011, raking in approximately $7.3 million in claims, nearly two-thirds of which were fraudulent.
The fraud was brazen. IBT Transportation billed Medicaid for patients who were already dead. They billed for individuals stuck in hospitals—nowhere near a ride. They billed for transport on dates patients were merely approved for service, regardless of whether a vehicle ever moved. At times, the company claimed to have transported more riders than their fleet could physically carry in a single trip—a logistical impossibility that slipped through the cracks for years.
Then came the mileage madness. State rules were clear: providers could only bill mileage for the first rider on a trip. Toran ignored the rules. He directed billers to switch tactics—sometimes billing no mileage, sometimes for every rider, and later, for every fourth. It was a moving target of deception designed to dodge detection while maxing out payouts.
The hammer came down thanks to a multi-agency takedown. The Illinois State Police Medicaid Fraud Control Bureau led the charge, backed by the U.S. Department of Health and Human Services Office of Inspector General, the FBI, and the U.S. Postal Inspection Service. The Illinois Department of Health and Family Services, which runs the state’s Medicaid program, provided critical data and support. The case was prosecuted by Assistant U.S. Attorneys Gregory K. Harris and Timothy A. Bass.
Toran now faces a five-year stretch behind bars. U.S. District Judge Sue E. Myerscough handed down the sentence on January 23, allowing Toran to remain free on bond until the Bureau of Prisons issues a self-report date. He must also pay full restitution: $4.7 million. The message is clear—rip off Medicaid, and the feds will come to collect, one way or another.
