Stefan Knoche, 55, of Lebanon, Pennsylvania, is facing federal charges for flooding the underground market with counterfeit drugs designed to mimic brand-name pharmaceuticals from Pfizer, Bayer, Eli Lilly, and Roche Holding AG. Authorities say the scheme ran for nearly a year, from May 23, 2017, to April 12, 2018, with Knoche knowingly pushing fake versions of Viagra, Aurogra, Xanax, Levitra, Cialis, and Valium—each bearing fraudulent trademarks of the real manufacturers.

The criminal information, unsealed by the U.S. Attorney’s Office for the Middle District of Pennsylvania, alleges Knoche didn’t just sell knockoffs—he trafficked them with the intent to deceive. According to U.S. Attorney David J. Freed, the drugs were marketed and distributed as legitimate, putting users at serious risk of harm from unregulated, potentially toxic substances. The counterfeit pills were often indistinguishable from the real thing, making detection nearly impossible for consumers.

Federal investigators from the U.S. Postal Inspection Service, FDA’s Office of Criminal Investigations, and Homeland Security Investigations collaborated on the case, following a digital and postal trail that led directly to Knoche. Packages intercepted in transit contained pills stamped with counterfeit branding, shipped through both domestic and international channels. Investigators say the operation relied on online sales platforms and encrypted communication to avoid detection.

Assistant U.S. Attorney James T. Clancy is leading the prosecution. If convicted, Knoche faces up to 10 years in federal prison, a term of supervised release, and steep fines. The Federal Sentencing Guidelines allow judges to weigh the severity of the offense, the defendant’s background, and public safety when determining punishment—meaning the actual sentence could vary significantly.

Authorities stress that counterfeit pharmaceuticals aren’t victimless crimes. Fake Xanax and Valium may contain fentanyl or other lethal fillers. Fake erectile dysfunction drugs could carry untested chemicals with dangerous side effects. This case underscores a growing black-market trend where profit is prioritized over human life—often with deadly results.

Indictments and criminal informations are formal allegations. Stefan Knoche is presumed innocent until proven guilty in a court of law. The case remains under active prosecution in the Middle District of Pennsylvania.
